Billing and Insurance

The New Jersey Out of Network Law requires our office to notify you, as a patient, that some of the services rendered by our staff may be provided on an out of network basis. You have the right to request a list of insurances that some of our providers are out of network with. You also have the right to request an estimate of charges and other pertinent information prior to receiving services. Our office is available to answer any questions you may have and you can also contact your insurance company for additional information.

When you register, we will ask you to sign an “assignment of benefits form,” or ABN, to authorize payment directly to this facility. We will file the claim to your insurance carrier after your procedure. If the insurance company sends payment to your home address, it is your responsibility to send the payment onward to the Center.

It is your right as a patient to request information or have questions answered regarding fee or payment policy. If you have any questions regarding your bill, please contact our business office between the hours of 8:30 a.m. and 4:30 p.m., Monday through Friday at 732-952-5533.
